For most people, medication is the right place to start. Here's a straight look at how the leading options stack up on evidence and side effects.

Finasteride or dutasteride?

Both lower DHT. Finasteride blocks the type 2 enzyme; dutasteride blocks type 1 and type 2, cutting DHT far more completely (~98% vs ~71%) and topping a recent network meta-analysis as the single most effective drug. The trade-off is that stronger suppression can bring more side-effect risk, and dutasteride is used off-label for hair in the US. A doctor can help you weigh it.

Minoxidil or finasteride?

They do opposite jobs, which is precisely why they're paired. Minoxidil grows hair by extending the growth phase and improving circulation; finasteride protects it by shutting off the DHT that shrinks follicles. Each helps alone, but together roughly 94% of men improved at a year in a 450-patient study.
